Loving mother of Mark (Janie) Jacobs, Gary (Christine) Jacobs and the late Robert, daughter in law Gail Jacobs, cherished grandmother of Kyle (Jose) Rivera, Scott (Fiance Leanne) Jacobs, Kevin (Beth) Jacobs, Tyler Jacobs, Jake Jacobs, adored great grandmother of Noah an Demi. Memorial contributions may be made to Vitas Hospice.
