Phyliss R. Lebowitz 90 of Boca Raton, FL. passed away on Sept. 17, 2014. Survived by her devoted Sons Dr. David (Patricia) Lebowitz and Dr. Michael (Carolyn) Lebowitz. Adored grandmother to Matthew, Nicole, Marc, Lauren, Jessica and Steven. Funeral services to be held Friday Sept. 19, 2014 9:30 AM at Levitt-Weinstein Memorial Chapel Fl. 7500 NO. State Road Seven Coconut Creek, FL. Interment to follow Eternal Light Memorial Gardens Cemetery. Shiva will be directly after service Friday only at Five Star Premier 22603 Camino Del Mar Apt. 1216 Boca Raton, FL.33434.
